The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y.

Jordan Health is now seeking a Woodward HIV Care Manager who, under the guidance of the Program Manager, is primarily responsible for providing follow up and confirming referrals and medical appointments. The WW HIV Care Manager will participate in case conferences, monitors patient services, conducts reassessments, provide crisis intervention, and participate in discharge planning for in-patient medical release and case closure when appropriate.

The WW HIV Care Manager will ensure to:

· Prepare, implement and monitors comprehensive Service Plan for patients.

· Educates HIV positive patients regarding services at Jordan Health and in the community and offers primary healthcare case management to patients.

· Advocates for patients’ access to entitlement programs, treatment facilities, and other needed community services.

· Conducts a comprehensive “patient needs assessment” to identify services required to meet patient needs.

· Participate in health fairs and outreach services in the Finger Lakes area when necessary.

Education And Experience Required:

· Bachelor’s degree in health, human or education services and a minimum of one year of qualifying experience in case management or casework with persons who are HIV+, have a history of mental illness, homelessness or chemical dependence.

Or an

· Associate’s degree in health or human services and two years of qualifying experience in primary health care setting working with HIV patients.

Licenses And Certifications Required:

· Provider BLS/CPR through the American Heart Association.

· Infection Control certificate.

Special Skills, Knowledge Required:

· Knowledge of the human services field and/or social work.

· Ability to develop an effective helping relationship, knowledge and experience in case management.

· Past experience in working with various ethnic and socioeconomic groups.

· Ability to speak Spanish and/or ASL is a plus.
