Here follows information about the position: :

Able Health Care Service is seeking a compassionate and dedicated Intake Specialist to join our team. As an Intake Specialist you are the initial point of contact for patients in need of Home Care. You are engaging, thorough, and possess great organizational skills. As you guide our callers through the process of starting Home Care Services with Able. Working hand and hand with the Billing/Authorization department is key for a smooth process.

- Excellent and Compassionate communication skills to ensure seamless transition into Home Care. Serving as primary point of contact during intake process.
- Evaluate callers and determine the types of service to be provided.
- Work in conjunction with Billing department to obtain necessary data to and from payor sources.
- Interpersonal skills working with MLTCs, Physicians, Health care professionals and families.
- Receive and discern all required information for accuracy and completeness of medical needs and patient demographics.
- Collaborate closely and transparently with Senior management, coordinators and clinical staff.
- Maintain accurate records and statistics regarding conversion rate of patient care, time frames of Initial call to Start of Care, and types of client care.

• Associate degree preferred.
• Bilingual Spanish a must
• 2-5 years' experience in Home Care, Intake, Hospice and CHHA.
• Proficient in operating HHA Exchange and HomeCare HomeBase.
• Proven success of developing and maintaining working relation with patients and advocates.
• Demonstrates initiative and leadership qualities
• Experience with authorization and insurance verifications.

- Annually $40,000 - $45,000 with potential growth incentives
