SUMMARY: The COMPASS Care Coordinator works as part of an interdisciplinary, patient-centered HIV care and prevention team to support the overall health and well-being of people living with or at risk of HIV/AIDS. They are responsible for providing direct care coordination services to assigned patient, to address barriers to treatment adherence, viral load suppression and retention in care. They assist the COMPASS Program Director and/or COMPASS Regional Director with managing the day-to-day functioning of their assigned program, ensuring the provision of high quality, patient centered care. They provide administrative supervision to assigned staff members. Must have experience working with marginalized populations, preferably with people living with HIV, mental illness and/or substance use.
