SUMMARY: The COMPASS Care Coordinator works as part of an interdisciplinary, patient-centered HIV care and prevention team to support the overall health and well-being of people living with or at risk of HIV/AIDS. They are responsible for providing direct care coordination services to assigned patient, to address barriers to treatment adherence, viral load suppression and retention in care. They assist the COMPASS Program Director and/or COMPASS Regional Director with managing the day-to-day functioning of their assigned program, ensuring the provision of high quality, patient centered care. They provide administrative supervision to assigned staff members. SUMMARY: Under the Cancer Screening Navigation Programming, the Health Education and Access Coordinator will help assess individual and community health education needs related to Prostate Cancer; plan, implement, and evaluate effective health education programs; coordinate health education services; serve as a resource person in health education. The goal of the Health Education and Access Coordinator is to provide people who are at high risk of Prostate Cancer with tools that allow them to prevent developing life-threatening health issues. Summary: Case managers screen children, individuals and families for insurance and other benefit program eligibility. Case managers will complete applications for eligible individuals/families. Case managers provide intensive tracking and follow-up with applicants to ensure that benefits are successfully accessed, and all required supporting documentation is obtained. Link patients to other resources in the community. Will work at multiple program site locations.
